Protected health information, about you, is obtained as a record of your contacts or visits for therapeutic counseling services
with therapist Brittany Bridges, LPC. Specifically, “Protected Health Information”(PHI) is information about you, including
demographic information (i.e., name, address, phone, etc.) that may identify you. PHI contains data about your therapy
past, present, or future health or condition, and the provision of health care therapy and counseling services to you.
In Cases of Abuse or Neglect and Serious Threats to Safety.
I may disclose your PHI to a public health authority that is authorized by law to receive reports of child abuse or
neglect. I may also use and disclose your PHI when necessary to reduce or prevent a serious threat to your health
and safety or the health and safety of another individual or the public. Under these circumstances, I will only
make disclosures to a person or organization able to help prevent the threat.
For Legal Proceedings.
I may disclose PHI in the course of any judicial or administrative proceedings, in response to an order of a court or
administrative tribunal (to the extent such disclosure is expressly authorized), in certain conditions in response to
a subpoena, discovery request or other lawful process.
Required Uses and Disclosures.
Under the law, I must make disclosures about you and when required by the Secretary of the Department of
Health and Human Services to investigate or determine my compliance with the requirements of the Privacy Rule.
